Introduction

To search for a provider near to you, select ADDRESS SEARCH. Enter your address and select the specialties you desire. Then click the SEARCH button.

Searching for a specific provider or medical group, use Name Search. Enter the name of the provider or medical group.

To find a provider in a specific city, county, or zip code, select Region Search.

To create a PDF directory of your search results, SELECT ALL/NONE to pick individual providers, or the entire search set. Once selected, hit the CREATE PDF DIRECTORY button.

Additional details about providers, click on the listing. A map and additional features and information will display.
